As busy parents, we understand how valuable your time is — that’s why we have made the intake process as easy as possible. We offer a variety of ways to get in contact with us to start the intake process. Additionally, with our Fusion patient portal, you can complete everything online at your convenience, allowing our team to focus on what matters most — getting your child the care they need as soon as possible!

We’re here to make the process as simple and stress-free as possible! Use whichever avenue is most convenient for you, and we will respond to your request within 24-48 business hours

Verify Benefits

If you have insurance, we will verify your benefits and eligibility as well as estimated costs of services. Please text or email a photo copy of the front and back of your insurance card to (984) 300-4002 or referrals@ccptnc.com. Upon request, we will also send a Benefit Summary Sheet.

Intake Forms

Our practice utilizes an online portal called Fusion to easily access and complete intake forms.

Please create a patient portal ON THE SAME DAY you are sent an email from Fusion. This link will expire if an account is not created in a timely manner. If you have any difficulties, please email admin@ccptnc.com.

Now you are ready to schedule!

A therapist will reach out to schedule an evaluation once the intake process is complete!

The evaluation includes a combination of standardized assessments and informal observations to assess your child’s skills. The therapist will also gather information about your child’s developmental history, strengths, and challenges through a parent interview.

Recommendations for therapy and/or home programs are made at this time.
